Edit Referrals

Who Can Edit Referrals?

The status of the referral determines who can edit it:

  • Submitted Status: External care providers use the Community Access® Portal to edit Submitted referrals.
  • In Progress Status: Patient placement specialists use the TransferCenterIQ™ application to edit In Progress referrals.

What Can Be Changed?

Who created the referral determines what can be changed:

Created by the Referring Facility

  • Submitted Status: All of the referral details can be changed if your referring facility created the referral, and it is in a Submitted status. Users with External Care Provider roles can make the edits.
  • In Progress Status: If the referring facility created the referral, and it is in an In Progress status, changes can be made to all of the referral details except clinical notes. Users with Patient Placement Specialist roles can make these edits using the TransferCenterIQ™ application.

Created by the Preferred Facility

  • In Progress Status: If the preferred facility created the referral, all of the request details can be edited by users with Patient Placement Specialist roles using the TransferCenterIQ™ application. Updates appear on the referring facility's Referral list and referrals details pages, as well as on the preferred facility's Cases list and case details pages.

Note:  

A referral no longer appears on the Referrals list if the patient placement specialist in the transfer centre changes the preferred facility to one that is not in your medical practice account.

When Can I Edit Referrals?

When a network connection is interrupted, you cannot edit referrals. The Edit Referral button on the View Referrals page and the Save Changes button on the Edit Referral page are disabled. Also, a message appears under the View Referrals and Edit Referral headings.

Another message briefly appears and the buttons are re-enabled when the integrations are re-established and you can edit referrals again.

Edit Submitted Referrals

  1. If you have an External Care Provider role, go to Access > Referrals to display the Referrals list,
  1. If the My Current Facility filter appears, select at least one facility in the list to display referrals from that facility. View the Referrals List.

  2. Select the edit icon in the first column of a referral that has a status of Submitted.

  2. If you have an External Care Provider role, make your changes in the appropriate fields as explained in the Create Referrals.
  1. Select Save Changes to save your changes and return to the Referrals page.

The changes to the referral appear in the Referrals list. The referral displays a value of Submitted in the Request Status column.

Edit In Progress Referrals

  • When a referral has a status of In Progress, it has been accepted into the destination facility's access centre and appears as a case. A Patient Placement Specialist can edit the referral as a case.
  • If you have a Patient Placement Specialist role, go to Access > Transfers to display the Cases list, and proceed as described in Edit Case Details.
