TransferCenterIQ™ Application Workflow

The typical work flow for the TransferCenterIQ™ application is as follows:

  1. The transfer centre receives a phone call or other communication requesting a patient transfer from a referring facility.
  2. The patient placement specialist opens a new transfer case and begins collecting information about the patient, the referring facility, and the referring consultant.

  3. The patient placement specialist contacts the on-call consultant for the requested specialty to possibly consult on the transfer case.

  4. The patient placement specialist conducts a conference call with the referring consultant and the on-call consultant to determine if the destination facility will accept the patient.
  5. If the facility accepts the patient, the on-call consultant becomes the accepting consultant and is responsible for the patient's care during the on-call consultant's shift.
  6. The patient placement specialist dispatches a transport company to pick up the patient if required.
  7. The patient placement specialist notes the transfer patient's arrival time at the destination facility.
  8. The patient placement specialist makes a bed request for the transfer patient and notes the time that the bed is assigned.
