What Does Importing Referrals Mean?

Importing referrals means that the requests are accepted from the Community Access® Portal into the TransferCenterIQ™ application. From there the referral can be processed as a transfer case. The patient can be admitted to the preferred facility or placed in a different facility.

Who Can Import Referrals?

A TransferCenterIQ™ specialist in the transfer center of the preferred facility can import requests from the Community Access® Portal using the TransferCenterIQ™ application if that application is integrated with the Community Access® Portal.

How Do Imported Referrals Display in the Community Access™ Portal?

When a patient placement specialist in the access center of a health system imports a referral into the TransferCenterIQ™ application on the TeleTracking IQ® platform, then the imported referral displays an In Progress status in the Referrals list of the Community Access® Portal.

Note:  

If the Community Access® Portal is integrated with the classic TransferCenter application, then the classic TransferCenter application user must have the following configuration in the Community Access® Portal:

  • A Patient Placement Specialist roles AND
  • Is mapped to a classic TransferCenter application user.

If both of the above conditions are not met, the imported referral continues to display a Submitted status in the Community Access® Portal after being imported by the transfer center.

How to Import Referrals into the TransferCenterIQ™ Application

  1. If your health system has the TransferCenterIQ™ application on the TeleTracking IQ® platform integrated with the Community Access® Portal and you have a Patient Placement Specialist role, select Access > Transfers to display the Cases list.

Note:  

If your health system has the classic TransferCenter application (on-premise) integrated with the Community Access® Portal, see the Integration with the Community Access™ Portal on the TeleTracking IQ® Platform section of the Classic TransferCenterIQ™ Application User Guide for more information about viewing and importing referrals.

  1. Select the Referrals tab to display the Referrals list.
  2. In the referral's first column, select one of the following icons depending on the action that you want to perform:
  • Select the Add to Case List icon.

The referral is accepted and appears on the Cases list. The referral no longer appears on the Transfers > Referrals list. The count of pending referrals in the Transfers> Referrals tab and browser tabs decreases by one. Referral clinical information appears as a Note in the Case Log with the external care provider as the author.

  • Select the Open Now icon.

The referral is accepted, appears on the Cases list, and the referral details appear in a new browser tab. The referral no longer appears on the Transfers > Referrals list. The count of pending referrals in the Transfers > Referrals tab and browser tabs decreases by one. Referral clinical information appears as a Note in the Case Log with the external care provider as the author.

External care providers see a status of In Progress on the Referrals list for the referral that was imported .
