TransferCenterIQ™ Application Workflow
The typical work flow for the TransferCenterIQ™ application is as follows:
- The transfer center receives a phone call or other communication requesting a patient transfer from a referring facility.
-
The patient placement specialist opens a new transfer case and begins collecting information about the patient, the referring facility, and the referring physician.
-
The patient placement specialist contacts the on-call physician for the requested specialty to possibly consult on the transfer case.
- The patient placement specialist conducts a conference call with the referring physician and the on-call physician to determine if the destination facility will accept the patient.
- If the facility accepts the patient, the on-call physician becomes the accepting physician and is responsible for the patient's care during the on-call physician's shift.
- The patient placement specialist dispatches a transport company to pick up the patient if required.
- The patient placement specialist notes the transfer patient's arrival time at the destination facility.
- The patient placement specialist makes a bed request for the transfer patient and notes the time that the bed is assigned.
